Getting An Appointment

New patients are seen after referral from their primary care doctor. Your doctor's office will send your records with the referral and they will be reviewed by our physicians to determine how soon you need to be seen. You will receive a phone call from our office staff to schedule your appointment.  

 

Before Your Appointment

Prior to your appointment, you will receive a reminder and your lab order will be faxed to your lab of choice. Please have the labs done at least 3-5 days prior to your visit so that labs will be available at your visit. Appointment reminders can be done by phone, email or text at your request.

If you need a personal medication card, you can make one here. Please be sure to bring a list of your medications to each visit.

Our secure patient portal is available for your convenience. By using MyCare, you can:

  • View your health summary from the electronic health record
  • View test results
  • Access health information resources
  • View upcoming appointments
  • Request medication refills

The Day of Your Visit

Please arrive early for your first visit. We want there to be ample time for you to find our office as well as time to make sure all of your paperwork is in order.  For your first visit, please bring: 

  • Government-issued photo identification
  • Insurance cards (including Medicaid and Medicare)
  • Your medications or a current medication list
